# Communication patients et proche aidants

Un professionnel de la santé ou une organisation peut inviter le patient ou des collaborateurs externes qui ne font pas partie du réseau Braver, mais qui gravitent tout de même autour du patient (ex. : parents, proches aidants), afin de communiquer et d'échanger.

1. Un patient ou proche aidant est toujours invité à partir d'une ou plusieurs [fiches patients](/pour-les-professionnels/fiches-patients.md)
2. Pour échanger avec ceux-ci, on créera des [canaux de soins](/pour-les-professionnels/canaux-de-soins.md) en les y invitant spécifiquement
3. Ces canaux de soins rassemblent ainsi toute l'information échangée avec le patient et ses proches de manière à les distinguer des autres canaux d'échanges interprofessionnels, ce qui permet d'éviter les erreurs
